Worst-case year = 12 × (monthly plan premium − any Part B giveback) + in-network maximum out-of-pocket, for covered Part A and B services. A giveback lowers the Part B premium you’d otherwise pay ($202.90 a month in 2026), so it counts as money back; a plan with a large giveback can show a worst-case year below $0. The Part B premium itself is the same for everyone, and drug costs are capped separately at $2,400 for 2027. Out-of-network care in PPO plans has a higher limit. Ties go to the lower premium. Other extras (dental, eyewear, hearing aids, over-the-counter allowance) are the yearly maximums each plan filed with CMS; they don’t change the order. Methodology

Plans without drug coverage
These 14 plans don’t include Part D. They mainly suit people with drug coverage from elsewhere, such as the VA or TRICARE. Joining one generally means you can’t also join a stand-alone Part D plan, and going without creditable drug coverage can trigger a late enrollment penalty.
