Medicare Costs: Premiums, Deductibles and Limits
Every standard Medicare cost in one place: Part A, Part B, Part D and the Medicare Advantage out-of-pocket limits, with the year each figure applies to and its source.
2027 figures: The 2027 Part B premium, deductible and IRMAA brackets have not been announced. CMS usually publishes them in mid-November. Part D and Medicare Advantage limits for 2027 are already final and shown below.
Part B (medical insurance)
| Cost | 2026 | What it means |
|---|---|---|
| Standard premium | $202.90 a month | Most people pay this. Higher incomes pay more (IRMAA). |
| Deductible | $283 a year | You pay this before Part B starts paying. |
| Coinsurance | 20% | Of the Medicare-approved amount for most services after the deductible, with no yearly limit in Original Medicare. |
Part A (hospital insurance)
| Cost | 2026 | Notes |
|---|---|---|
| Premium | $0 for most people | If you or your spouse paid Medicare taxes for 40 quarters. Otherwise $311 a month (30–39 quarters) or $565 (under 30). |
| Hospital deductible | $1,736 | Per benefit period, not per year. Covers days 1–60. |
| Days 61–90 | $434 a day | Coinsurance per benefit period. |
| Lifetime reserve days | $868 a day | After day 90; 60 reserve days over your lifetime. |
| Skilled nursing facility | $217 a day | Days 21–100 of a covered stay; days 1–20 are $0. |
Part D (drug coverage)
| Cost | 2026 | 2027 |
|---|---|---|
| Maximum deductible | $615 | $700 |
| Out-of-pocket cap | $2,100 | $2,400 |
| National base beneficiary premium | $38.99 | $41.33 |
Plans can set a lower deductible. The base beneficiary premium is used to calculate the late enrollment penalty; your plan’s premium is set by the plan.
Medicare Advantage out-of-pocket limits
Every Medicare Advantage plan must cap what you pay for covered Part A and B services. These are the highest limits CMS allows; many plans set lower ones.
| Limit | 2026 | 2027 |
|---|---|---|
| In-network maximum | $9,250 | $9,850 |
| Combined in- and out-of-network (PPO) | $13,900 | $14,800 |
Part D drug costs are capped separately (above). Premiums don’t count toward either limit.
Questions
How much is Medicare Part B in 2026?
The standard Part B premium is $202.90 a month and the deductible is $283. People with higher incomes pay more through IRMAA.
Is Part A free?
For most people, yes. If you or your spouse worked and paid Medicare taxes for at least 40 quarters (about 10 years), you pay no Part A premium. You still pay the hospital deductible and coinsurance.
Is there an out-of-pocket maximum in Original Medicare?
No. Original Medicare has no yearly limit on the 20% coinsurance. Medicare Advantage plans and Part D have caps, and a Medigap policy can cover most of the gap.
When are the 2027 Part B figures announced?
CMS usually publishes the next year’s Part A and Part B premiums and deductibles in mid-November. We update this page when they are released.
