How IRMAA works

The income-related monthly adjustment amount (IRMAA) is a surcharge on Part B and Part D premiums for people with higher incomes. Social Security decides it each year using the modified adjusted gross income (MAGI) on your tax return from two years earlier, so your 2026 premiums are based on your 2024 return. MAGI here means adjusted gross income plus tax-exempt interest.

It is a cliff, not a slope: one dollar over a threshold puts you in the higher bracket for the whole year. That is why the calculator shows how far you are from the next bracket. If you’re close, things like Roth conversions, capital gains or a large IRA withdrawal two years before can matter.

Questions

What income is used for IRMAA?

Your modified adjusted gross income from the tax return two years before the premium year: adjusted gross income plus tax-exempt interest. Social Security gets it from the IRS.

Can I appeal IRMAA?

Yes. If your income went down because of a life-changing event such as retirement, reduced work hours, marriage, divorce or the death of a spouse, you can ask Social Security to use a more recent year with Form SSA-44. You can also request a reconsideration if the IRS data was wrong.

Does IRMAA apply to Medicare Advantage?

Yes. Medicare Advantage members still pay the Part B premium, including any IRMAA, and the Part D surcharge applies if the plan includes drug coverage.

When will the 2027 brackets be published?

CMS normally announces the next year’s Part B premium and IRMAA brackets in mid-November. We update this calculator when they are published.
