Lifetime Medicare Cost Calculator: What Medicare Costs From 65 to 90
Add up Part B, Part D, a Medigap or Medicare Advantage plan, your share of care and any IRMAA surcharge, year by year. The starting point is today’s official figures. How fast costs grow is your assumption, and you can change it.

Year one uses the latest official figures on file: the 2026 Part B premium, deductible and IRMAA brackets and 2026 Part A premiums from CMS, and the 2027 Part D and Medicare Advantage out-of-pocket caps. On Advantage, the plan’s own cost sharing (the care costs you enter) replaces the Part B deductible. Every later year grows by the rate you enter; IRMAA brackets are assumed to grow too, so your bracket stays the same. These are projections from your assumptions, not official figures. Totals are for one person, in future dollars. Nothing you type leaves your browser.
What goes into the total
- Part B: the standard premium ($202.90 a month in 2026), the yearly deductible ($283) on Original Medicare, plus IRMAA if your income is above $109,000 (single) or $218,000 (joint).
- Part A: premium-free for most people with 40 quarters of work. Otherwise $311 or $565 a month in 2026.
- Drugs: your Part D premium (or the drug coverage in your Advantage plan), the Part D IRMAA surcharge, and what you pay at the pharmacy, which can’t pass the yearly cap ($2,400 in 2027).
- Medigap or Medicare Advantage: the premium from your quote. Medigap pays most of what Original Medicare leaves; Advantage plans charge copays up to a yearly out-of-pocket limit ($9,850 in network at most in 2027).
It leaves out dental, vision and hearing (Original Medicare covers little of them), long-term care, which Medicare doesn’t cover, and care outside your plan’s rules.
About the growth rate
Medicare figures change every year. CMS sets the Part B premium each fall, Part D limits are indexed each year, and Medigap premiums depend on your insurer and how it prices by age. Nobody publishes official projections for 25 years out, so this calculator asks for your own growth rate. Try a low and a high rate to see the range. Our Medigap pricing guide explains why some policies rise faster with age.
Questions
Is this in today’s dollars?
No. The totals are in future dollars, grown by your rate each year. To see a rough figure in today’s dollars, set the growth rate to how much faster you expect health costs to grow than prices in general.
Is Medigap or Medicare Advantage cheaper over a lifetime?
It depends on your premiums, your health and where you live. Advantage often costs less in healthy years and more in years with a lot of care. Our county comparison uses CMS plan data to compare both for one year.
Why does IRMAA matter so much?
It’s charged on top of both Part B and Part D for every month you’re in a bracket. Over many years, lowering income in the years that count can save more than switching plans.
